PYA Scholarship & Sponsorship Assistance Request

Pacific Youth Association believes every child should have the opportunity to participate. This form allows families to request confidential financial assistance for registration fees when needed.

Requests may be approved as full or partial assistance
Assistance is based on available funds and current demand
Submitting this form does not guarantee approval
You may be contacted for clarification or follow-up if needed

Confidentiality

Information submitted through this form is reviewed privately by a small scholarship review group and is used only to evaluate and process this request. Details are handled discreetly and respectfully.

Timing & review

Please submit requests as early as possible. Most requests are reviewed within 2–5 business days, though processing may take longer during peak registration periods.

Please answer honestly. Our goal is to support families while maintaining fairness and sustainability across PYA programs.

Briefly describe the circumstances leading to this request. Examples include temporary financial hardship, medical expenses, job changes, or multiple children registered. You are not required to share personal details beyond what you are comfortable.

This helps PYA understand how to best allocate limited scholarship funds.

Has your child participated in PYA previously?

“Yes” if your child has played in any prior PYA season.

Will your child attend practices and games.

This helps ensure assistance is used for active participation.

Volunteering is appreciated but not required. Volunteers can earn credits to use towards registration.
